3. Provide effective physician directed case management for patients with multiple chronic problems that have manipulated his or her health.
4. Guide patients through health care delivery systems.5. Reduce the number of unsuccessful and unnecessary hospital admissions and readmissions.

We are a physician lead organization based in Chandler, Arizona. Our network of medical providers aims to provide fully integrated care and services to surrounding communities in Arizona. Arizona Priority Care is owned by Heritage Provider Network (HPN), allowing us to connect with affiliated medical groups across the United States. We are equipped with a team of physicians, specialized nurses, social workers, and other highly trained professionals with one consistent goal in mind: delivery timely, compassionate, and comprehensive medical care.

Our Care Centers are state-of-the-art facilities designed for the patient. The physicians at our Care Centers are available when your primary care physician (PCP) is unavailable. Our Care Center procedures work in tandem with your regular PCP to help meet your immediate health care needs.
