Complex Care Management (CCM)
Our CCM program assists the Primary Clinician with managing the care of patients with multiple & complex medical conditions. In this program, a Registered Nurse Care Manager (RN CM), a Licensed Clinical Social Worker (LCSW), you and your family, as well as your Physician(s) work together to achieve your best health possible. Our RN CM will come to your home for a face-to-face visit, create a care plan and will follow up with you monthly thereafter. They will keep your Primary Clinician and your Specialist(s) up to date on any identified issues. Also, our LCSW will work with you for any needs you may have, including completion of Advance Directives, help with applying for Medicaid, Long Term Care, or other social related needs and resources.
Telephonic Care Management (TCM)
Telephonic Care Management (TCM) is a program for members who do not require intense interventions, but may need some additional guidance such as referrals to community resources, finding a doctor or monthly check-ins over the phone with one of our nurses. Our TCM program serves as a means of encouraging wellness through frequent communication and ongoing healthcare education.
Palliative Care (PC)
Palliative Care focuses on controlling your pain and symptoms related to a chronic disease condition. The team that provides care to you includes a Nurse Practitioner, RN or LPN, Certified Nursing Assistant, Social Worker, and a Chaplain.
Social Services
The primary goal of Social Services is to provide you with information, resources, and ongoing support. Our Licensed Social Worker will assist you with problem solving the social issues that affect your overall health. Our social worker will provide education and assistance in completion of Advance Directives such as Durable Power of Attorney, Medical Power of Attorney, or a Living Will. We can also help you apply for Medicaid and Long Term Care Services.
