Transitional care refers to the coordination and continuity of health care during a movement from one healthcare setting to another or to home. This is called “Transitions of Care”.
Arizona Priority Care employs Nurse Practitioners and other health care professionals that develop and plan for you to address your acute or chronic illness. The goal of this team is to become familiar with you and follow-up with you once you are out of the hospital or Skilled Nursing facility to address any barriers that may prevent you from reaching your highest level of health possible. The types of services that we can provide:
- Schedule a follow-up visit with one of our NP’s, Complex Care Managers and or a licensed social worker
- Review medications
- Perform a physical assessment
- Communicate with your primary clinician
- Provide community resources
- Provide help completing Advance Directives, (social work)
