How to Become a Provider

Thank you for your interest in joining Arizona Priority Care (AZPC). AZPC’s strives to contract with highly qualified independent providers to deliver the best care to our members.

• If you are interested in becoming an AZPC network provider, please complete the New Practice Group Participation Request form.

• If your practice is currently participating with AZPC and you wish to add a provider to your existing Agreement, please complete the Existing Practice Group Participation Request form.

Please allow up to thirty (30) business days to receive a response to your request. We will notify you of the status of your request and may need additional information once our review is completed.
