Advance Directives
You should talk with others about your wishes so those closest to you know what medical treatments you may or may not want if you are incapacitated or if you are approaching the end of your life. Life planning can include creating a Living Will, assigning a Health Care Power of Attorney, or a Do Not Resuscitate (DNR) order.

Area Agency on Aging – Programs and Services
Your local Area Agency on Aging offers multiple programs and services, including resource guides for seniors, adult day care, employment, AmeriCorps, Medicare benefits assistance, care transitions, meals, family and caregiver support, healthy living classes, legal assistance, independent living, and mental health resources.

COVID – 19
Arizona Priority Care provides a COVID-19 Care program for you if you test positive for the coronavirus. We offer a virtual visit with a Nurse Practitioner (NP) and links to resources or printed materials (available in English and Spanish) on what to do if you are sick or caring for someone sick. We will remain in contact with you throughout the course of your illness and offer telephonic support for as long as necessary.

Life Planning Resources
Did you know that you can authorize someone else to act as your Representative to discuss your medical care on your behalf? Your Representative will be allowed access to your Protected Health Information (PHI) to communicate with Arizona Priority Care and execute any other HIPAA-related patient rights. However, this does not allow your Representative to make any healthcare decisions on your behalf. To learn more about how to do this, please call our Customer Service Advocates at 480-336-7444.
