Arizona Priority Care (AZPC) is an Integrated Provider Network focused on providing whole-person care to Senior and Medicaid populations, through advanced value-based models. Under the supervision of the Vice President – Clinical Services, the Nurse Practitioner (NP) will provide direct patient care to AZPC members to improve outcomes in quality, patient experience, and help mitigate utilization of resources in support of AZPC organizational goals and objectives. The NP shall perform autonomously with an emphasis on disease prevention and health management. This position is for the West Valley.
POSITION DUTIES & RESPONSIBILITIES
· Performs quality patient care within NP score of practice and ensures delivery of effective and appropriate care consistent with established regulatory, licensing, and evidence-based practice standards.
· Performs patient annual wellness visits in a variety of settings to include, but not be limited to, the patient’s private residence, assisted living facility, and primary care provider office, specialist office, hospital, or skilled nursing facility.
· Conducts chart review to assist in supporting proper diagnoses, status and plan of care specific to patient acute and chronic conditions.
· Assesses patient for all acute and chronic conditions and diagnoses each condition that impacts the patient’s care and treatment.
· Adheres to documentation standards and completes documentation of assessed acute and chronic conditions with the following documentation components within timeframe outlined by AZPC guidelines
• Correct diagnosis code
• Status of condition
• Provider plan to address each diagnosis code
· Demonstrates knowledge and proficiency in understanding key elements of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) model measures and how these relate to AZPC organizational goals and objectives within 90 days of hire.
· Demonstrates knowledge and proficiency in understanding key elements of Quality STAR (HEDIS) measures and how these relate to AZPC organizational goals and objectives within 90 days of hire.
· Educate and counsel patient/family on preventive services and ordered recommended screening tests applicable (e.g. DRE, A1c, DEXA, Mammogram, Colorectal screening).
· Demonstrates knowledge and proficiency in understanding of co-existing disease condition interaction and properly diagnoses and assigns diagnosis codes.
· Educates and guides patients on disease prevention and health promotion throughout the lifespan with particular focus on aging process, geriatric population health.
· Promotes a safe environment. Arizona law mandates that certain professionals report abuse, neglect, or exploitation if they have a basis to believe it has occurred.
· Notifies patient and/or designee of any recommended changes to plan of care.
· Prescribe, order, administer and dispense therapeutic measures including pharmacologic agents and devices in accordance with Arizona State Board of Nursing Advance Practitioner rules and regulations.
· Makes therapeutic adjustments and/or recommendations to medications as needed.
· Orders, performs, and interprets diagnostic tests and screening evaluations such as lab work, diagnostic imaging, spirometry, ankle/brachial index (ABI) and point of care testing and provides timely follow up communication to patient of results.
· Works collaboratively and cooperatively with members of AZPC organization and other agencies to coordinate care and services required by patients.
· Maintains knowledge and proficiency in medical practices through continuing medical education (CME).
· Participates in development if the AZPC Home Wellness Program to meet the health care needs of AZPC members.
· Participates in and contributes to HWP staff orientation, education and development upon request.
· Participates in the interview and selection process of HWP staff upon request.
· Functions as internal resource for clinical and non-clinical team members to promote positive patient / customer outcomes.
· Other duties as directed.
EDUCATION, TRAINING AND EXPERIENCE
· Master’s Degree in Nursing
· Board Certification as Advanced Practice Nurse from a nationally recognized certifying body as required by the Arizona State Board of Nursing.
· Family Nurse Practitioner or Adult Nurse Practitioner required. Geriatric experience preferred.
· Current unrestricted State of Arizona RN License.
· Current Arizona Advanced Practice Nursing license; temporary license will be accepted until receipt of permanent licensure pending national board certification.
· Full unrestricted prescriptive and dispensing privileges with DEA licensure required.
· Current basic CPR certification or ability to obtain within 30 days or hire.
· Flexibility in their work environment to travel throughout the valley is required.
· Self-directed and must have excellent organizational skills.
· Excellent interpersonal communication (written and verbal) skills.
*This role requires FT in-office presence for the first 60 days of employment. Hybrid schedule available after initial training period.*
